Spiritual Direction Agreement
Our Relationship
You are meeting with me as your spiritual director because you want to examine your spiritual life. I am here to hold compassionate space, engage in holy listening, and ask questions. I believe it is appropriate at times to discuss psychological and relational difficulties in the context of spiritual direction. However, I am not a licensed counselor or therapist, nor do I provide that service. Spiritual direction can be wonderfully fruitful and also can be challenging both mentally and emotionally. Spiritual direction is not considered psychotherapy, mental health counseling, or otherwise a therapeutic service.
Confidentiality and Privacy
I will hold our conversation in confidence. Our time together is sacred. Obligation to report: Texas state law requires me to report suspected cases of child or elder abuse. Also, professional ethics regarding confidentiality apply to suicidal/homicidal ideation or any other situation that puts your or other’s health and safety in danger. A court order can require me to release information contained in my records or require me to testify in court.
Payment and Cancellations
The fee per a 55-minute session is $80, payable by the end of the session. If you need to cancel or reschedule an appointment, please let me know by email or phone at least 24 hours in advance, except in cases of emergency. For cancellations on the day of the session, I ask that you still pay for the missed session.
By typing my name below, I acknowledge that I have read and understand the above conditions.
Supervision and Confidentiality Release Form
It is customary for spiritual directors to be in supervision as a part of their own spiritual and professional development. Emily Phillips Davis, as my spiritual director, has my permission to occasionally use material from our spiritual direction sessions for her own professional supervision. I understand that my name will never be used, that the situation will be disguised, and that the supervision focus is on the director, not me, the directee (client).
If in the course of spiritual direction, I am also working with a psychotherapist, counselor or other professional, I give permission for Emily Phillips Davis to discuss my care with my therapist if it seems necessary for my well-being. I understand that Emily will inform me before speaking with any professional about my care and will keep this information confidential.
